This is Mega Scrafty EX, card 110/193 from the Japanese Mega Dream EX set. Scrafty is a fan favourite from the Unova region, and seeing it presented in its Mega Evolved form gives the card real character. As a Japanese print, it features the artwork and layout as released for that set.
The card is a Double Rare, shown by the pair of black star symbols that mark ex cards in this era. Double Rare ex cards highlight some of the more powerful Pokemon in a set and sit above the standard rares, which makes them a natural target for collectors building a set or chasing a specific Pokemon.
